Grooming Contract

This grooming contract only needs to be signed once per owner. Once on file you will not have to sign or turn in again. 

Most animal vaccination records are good for 1 year (some are 3 year). We will keep track and file of records but it is the owners responsibility to keep us updated with the newest records to keep on file. Riverbend Pet Spa requires 48 hrs notice for any cancellation or reschedule of appointments. If you cancel or reschedule within 48 hrs, you will be charged a fee of 50% of the service booked. This must be paid before rescheduling. If a client misses or reschedules 3 times, they will be moved to pre pay only and must pre pay for all services at the time of booking plus any late fees associated with missing appointments. If the miss future appointments that are pre paid, there is no refund and Riverbend Pet Spa keeps 100% of the price of the groom that was missed. No credit or refund will be given. If your pet is not up to date on shots at the time of appointment or you can not provide proof of that, you will be charged the cancellation fee. 

Please E-mail us any updated vaccination records and or signed grooming contract before your appointment. The following up to date immunizations must be on file with Riverbend: Distemper, Pavovirus (also commonly known as the DHPP or DA2PP +L4 vaccination), and Rabies. Bordetella (kennel cough) is not required but recommended. Up to date flea and tick preventative is also recommended but not required.
